What to Bring

Insurance cardBring your current insurance card. If you have more than one plan (primary and secondary), bring both.

Photo IDA government-issued photo ID is required for all new patients.

Referring provider informationIf you were referred by a physician, bring the referral paperwork or the referring provider's name and contact information.

Prior records and test resultsAny relevant labs, culture results, imaging reports, or prior visit notes. If you are being seen for a post-ER or post-urgent care follow-up, bring your discharge paperwork.

Current medication listA complete list of all medications you are currently taking, including dose and frequency, and any antibiotics you were prescribed at your prior visit.
Billing

Understanding Your Bill

Copays and coinsurance may, if applicable, need to be paid at the time of your visit. After your visit, we will submit a claim to your insurance on your behalf. Your insurance will process the claim and apply any deductibles according to your plan. You will then receive a statement for any balance that remains after insurance payment.

You can view your balance and pay online through our patient portal at any time. If you have questions about a bill or need to discuss a payment arrangement, please call our billing office during normal business hours.

Self-Pay Options

For patients whose insurance is not in-network or who have no insurance and still wish to be seen, please call our office to discuss billing arrangements before scheduling your appointment. We will work with you to ensure you understand the expected costs and payment options available.
